Self-medication is a globally widespread health practice that involves the consumption of drugs without prior clinical assessment or professional prescription, posing significant risks to patient safety. In rural communities of Ecuador, this behavior acquires particular characteristics due to limited access to health services, geographic dispersion, and the persistence of traditional health knowledge. The objective of this research was to analyze the factors associated with self-medication in rural communities of Ecuador from the perspective of nursing education, through a review of the scientific literature. The methodology consisted of a descriptive literature review, searching the SciELO, PubMed, LILACS, Dialnet, Google Scholar, and Redalyc/CENIC databases. Through a selection process based on inclusion and exclusion criteria, 14 studies published between 2018 and 2026 were identified. The findings were organized into five categories: sociocultural factors and ancestral knowledge, barriers to accessing health services, antibiotic self-medication and antimicrobial resistance, sociodemographic and economic factors, and the role of nursing education. The results showed that distance to health facilities, low educational attainment, the influence of traditional medicine, and easy access to over-the-counter drugs are the main determinants of self-medication in rural settings. It is concluded that community nursing plays a strategic role in promoting the rational use of medicines through culturally relevant educational interventions aimed at strengthening health literacy among rural populations.
