| dc.description | Post-inflammatory hyperpigmentation (PIH) is an acquired pigmentary disorder that develops following inflammatory processes, skin injuries, or dermatological procedures and may negatively affect quality of life, particularly in individuals with darker skin phototypes. This review aimed to analyze the effectiveness and safety of tranexamic acid (TXA) as a depigmenting treatment for PIH. A narrative, descriptive, and comparative literature review was conducted using PubMed, SciELO, and Google Scholar. Publications in English and Spanish from 2021 to 2026 were prioritized, while relevant classic sources were retained for the theoretical framework. The evidence identified was limited and heterogeneous. Topical 5% TXA showed efficacy comparable to 20% azelaic acid for acne-related PIH, with better initial tolerability. Intradermal administration showed promising results for treating established lesions and preventing PIH following laser procedures; however, treatment protocols were not standardized. Evidence regarding oral TXA was mainly derived from small studies and combination treatments; therefore, its independent effect could not be determined, and a greater potential for systemic adverse effects was identified. In conclusion, TXA represents a promising therapeutic alternative, particularly when administered topically and, in selected patients, intradermally. Its use should be individualized and accompanied by control of the underlying inflammation and adequate photoprotection. Larger clinical trials and standardized protocols are required to determine the optimal concentration, route of administration, and treatment duration. | |
| dc.description.abstract | La hiperpigmentación postinflamatoria (HPI) es una alteración pigmentaria adquirida que aparece después de procesos inflamatorios, lesiones cutáneas o procedimientos dermatológicos y puede afectar la calidad de vida, especialmente en personas con fototipos altos. El objetivo de esta revisión fue analizar la efectividad y seguridad del ácido tranexámico (ATX) como tratamiento despigmentante para la HPI. Se realizó una revisión bibliográfica narrativa, descriptiva y comparativa en PubMed, SciELO y Google Scholar. Se priorizaron publicaciones en español e inglés correspondientes al periodo 2021-2026 y se conservaron fuentes clásicas relevantes para el marco teórico. La evidencia identificada fue limitada y heterogénea. El ATX tópico al 5% presentó una eficacia comparable al ácido azelaico al 20% en la HPI posterior al acné, con mejor tolerabilidad inicial. La administración intradérmica mostró resultados prometedores para tratar lesiones establecidas y prevenir la HPI posterior a procedimientos con láser; sin embargo, los protocolos no fueron uniformes. La evidencia sobre el ATX oral procedió principalmente de estudios pequeños y tratamientos combinados, por lo que no fue posible determinar su efecto independiente y se identificó una mayor posibilidad de efectos adversos sistémicos. En conclusión, el ATX constituye una alternativa terapéutica prometedora, principalmente por vía tópica y, en pacientes seleccionados, intradérmica. Su empleo debe individualizarse y acompañarse del control de la inflamación y fotoprotección. Se requieren ensayos clínicos mayores y protocolos estandarizados para establecer la concentración, vía y duración óptimas. | |
